Fathom and eClinicalWorks both turn the documented encounter into the visit codes. A practice already on eClinicalWorks can retire the standalone Fathom contract and move coding into the EHR it already licenses: use eClinicalWorks' native Clinical Rules Engine (CRE), which fires pre-configured rules off the clinical documentation (lab values, diagnoses, vitals) and auto-drops the appropriate CPT/ICD-10 codes straight into the progress note and the integrated charge/claim workflow, plus E&M / Level-of-Service code recommendations and the in-context eva assistant during documentation. Because the codes attach directly to the note, orders, and billing record the clinician already produced, there is no export-out-and-post-back loop the way a third-party engine like Fathom needs. Keep eClinicalWorks (the system of record); cut Fathom.
- Warning: eClinicalWorks' native coding (CRE / E&M recommendations) is rules-based and clinician-in-the-loop, not the fully autonomous 'direct to bill, no human intervention' automation Fathom sold — fully autonomous GenAI chart coding on eClinicalWorks is largely a third-party add-on, so expect coders to stay in the loop after cutover and re-validate coverage on your specialties before dropping Fathom.

- Warning: Fathom also runs retrospective risk-adjustment (HCC/RAF) coding (node:fathom-health/risk-adjustment-coding); eClinicalWorks' built-in CRE/E&M coding does not clearly cover that exhaustive retrospective HCC use case, so confirm a replacement path before cutover if the practice relies on it.
